1) You know whom to call for information regarding aging issues.
2) You have identified a primary person and a backup to call in an emergency.
3) You have prepared to the greatest extent possible for a natural disaster.
4) You have a relationship with your physician, and you partner with him in your care..
5) You are taking steps to improve all aspects of your health so that you are able to function at a level that is satisfactory to you.
6) You have advance directives in place for end-of-life care (including funeral, will, medical directives and DNR).
7) You have discussed all your end of life wishes and estate issues with your family.
8) You have a plan for how to spend your time (includes recreational activities, hobbies, volunteer and educational opportunities.
9) You have a financial plan in place to meet your long term care needs.
10) You have taken steps to modify your current housing to meet your long term needs or know where you want to live if you need additional assistance.
11) Request the information you need and then discuss the options with someone who can help you make informed choices.
